Disney Trip Paperwork and Link

To sign up for the Disney trip, students must pay a $200 non-refundable deposit to the bookkeeper and return four signed forms (trip consent, amusement park consent, TSA consent, and the newly added Medication Authorization Form), plus attend 5 of 6 rehearsals starting March 24th.

Dear Prospective Touring Ensemble Members and Families,

To sign up for the Disney trip, all students must

  1. Make a $200 non-refundable downpayment to our school bookkeeper: https://wa-snohomish.intouchreceipting.com/, and
  2. Sign and return the attached paperwork. There are 4 documents to turn in: a trip consent form an amusement park consent form, and a TSA-required consent form on the last page of the itinerary. a Medication Authorization Form

(Please note, the Medial Authorization Form was added Wednesday, October 30th. If your paperwork was turned in prior to that date, please also turn this form in to Mr. Morris.)

By submitting the deposit and paperwork, your student is agreeing to the following:

  1. Attend 5 of the 6 scheduled rehearsals beginning March 24th (https://www.grizzlyband.org/calendar/).
  2. Follow through on their commitment to participate in the activities outlined in the itinerary.
  3. Follow the guidelines and expectations outlined in the Behavior Expectations document.
